Privacy Policy

Nicola Cherry Physiotherapy is committed to protecting your privacy and personal information.  This policy specifies how your personal data is collected, used and stored.  Collection and process of your personal data is a legal obligation, governed by the Chartered Society of Physiotherapy and the Health Professions Council and compliant with the General Data Protection Regulations (EU) GDPR 25.05.2018.  This policy is effective from 01/11/18.

What data is collected?

On contacting and making an appointment with Sussex Performance Physiotherapy, you will be asked for the following data;

  • Name

  • Date of Birth

  • Address

  • Email address

  • Telephone Number

On your initial assessment, you will be asked for the following information:

  • Current medical health

  • Past medical history

  • Medication you are taking

  • Results of any scans or investigations.

  • Leisure and activities of daily living.

  • Employment and current work requirements.

  • Special questions relating to the condition you are seeking treatment for. 

Photographs may be taken as a component of postural assessment.

How long is your data kept?

There is a legal duty to store your data for 8 years.  After this time your data is destroyed.

How is your data used?

The data gathered, along with the results of your physical examination will be used to assist in your diagnosis and treatment planning.

With your consent, your data may be shared with other medical professionals involved in your care, such as GP's, consultants and insurance providers.  In addition, your data may be disclosed if required to do so for legal purposes.

Your data may also be used for internal audit purposes.

Your data will not be shared with any third parties (except as detail above) or commercial services.

How is your data stored?

Appropriate electronic and security measures have been taken to secure your data to prevent the unauthorised access and disclosure of your data.  Paper records will be stored with due diligence in locked storage cabinets.
